WHAT IS PILES?

Piles (Haemorrhoids) is the formation of lumps of swollen veins and tissues in or around the anus or lower rectum area that lead to significant discomfort.

Haemorrhoids may not cause any symptoms in certain individuals while in others, it leads to bleeding after passing stool, a lump hanging from the anal opening, abnormal mucus discharge, and swelling around the anus.

A person should see the General physician immediately if the above complications persist, like continuous rectal bleeding.

TYPES OF PILES

External Piles

Can occur beneath the skin surrounding your anus

1
Internal Piles

Can occur inside your rectum and anus.

2

CAUSES

  • Lifting heavy weight
  • Being pregnant
  • Having anal intercourse
  • Chronic diarrhea or constipation
  • Difficulty and straining while passing stool

HOW IS IT FORMED

The constant pressure in the lower rectum is one of the major causes leading to piles. When an individual consistently exerts pressure near the anus and rectum while lifting heavy weights or constipation, then the blood vessels near the anus become swollen and bulge. This further leads to the formation of piles.

SYMPTOMS

  • Bloody Stool
  • Painful lump around the anus
  • Pain while passing stool
  • Discomfort in the anal area while sitting
  • Pain while lifting heavy weights
  • Feeling of unclear bowel even after passing stool
  • Fatigue

DIAGNOSIS

An anorectal surgeon will diagnose the external piles while in internal hemorrhoids rectum and anal canal are examined. Two common tests which are done to diagnose piles are enlisted below:

Visual inspection: If the internal hemorrhoids are too soft to be felt, the piles' doctor might closely examine the lower rectum using an anoscope, proctoscopy, or a sigmoidoscopy. Using these devices, the doctor will carefully examine the anal tissues of the anus lining to check the severity, grade, and growth of piles.

Digital examination: In this diagnostic examination, the piles specialist inserts a finger into the rectum using a glove. After lubricating it after inserting the finger, the proctologist checks for any kinds of abnormal growth.

DO's
Drink Sufficient Water
Increase fiber intake
Exercise regularly
Stay physically active
DONT's
Don't ignore bowel movements
Avoid sitting on toilet seat for long
Avoid having spicy foods
Abstain from heavy weight lifting

TREATMENTS

NON SURGICAL TREATMENTS

If piles is not that severe, doctor might suggest a non-surgical treatment for piles. The two most popular non-surgical treatments to cure piles are:

  • Rubber band ligation: During this procedure, the proctologist places a rubber band around the base of the internal piles. The rubber band cuts off the blood flow to the piles tissues causing it to shrink and fall off within 5-7 days.
  • Coagulation: A small probe that transmits heat to destroy the hemorrhoidal tissue is called infrared coagulation.
  • Electrocoagulation: The procedure that involves use of electric current to stop the blood flow to the hemorrhoid.
  • Sclerotherapy: The procedure that involves injecting a chemical into the swollen vein of hemorrhoidal tissue.

SURGICAL TREATMENTS

  • Hemorrhoidectomy (Traditional Surgery): This surgical procedure is performed to eliminate the protruded anal tissues that cause bleeding. It is suggested by the surgeons when the piles gets severe.
  • Hemorrhoid stapling: The procedure that involves blocking of blood flow to hemorrhoidal tissue by a stapling instrument is called hemorrhoid stapling . This procedure is comparatively less painful than hemorrhoidectomy.
  • Laser surgery: This surgical procedure uses laser beam to shrink or even burn the inflamed hemorrhoidal tissues. The surgeon focuses the laser beam on the affected tissues to obstruct the blood flow and destroy it.

RISKS AND COMPLICATIONS

Some of the risk factors associated with piles are:

  • Chronic constipation
  • Obesity
  • Pregnancy

Complications that may arise in piles include:

  • Infection
  • Anemia
  • Strangulated tissue (cut off blood supply to the hemorrhoidal tissue)

IF LEFT UNTREATED

Piles can get strangulated if left untreated, thereby cutting off the blood supply to the tissues. Due to this, the person might experience venous thrombosis within the piles leading to extreme pain. Also, since the major symptom of piles is bleeding, with time it can lead to anemia and other complicated health hazards.

COST

The cost of piles treatment is between 30,000 INR to 1,20,000 INR in India. However, the cost of this treatment also depends on the type of surgery, procedure, facilities provided by your chosen hospital, etc.

INSURANCE COVERAGE

Piles treatment is mainly covered under your health insurance. Most of the time, your insurer will cover a particular amount for piles and related diseases. However, the amount of coverage you will have depends on the type of policy.

It is important to know that medical treatment for piles will not be covered under your insurance policy, it will only cover the cost or expenses of the surgical treatment, but to claim this, you will have to be hospitalized for 24 hours.
